A program offering education and support for expectant mothers.
Chronic Condition Management
This program has recently changed to a more robust case manager-driven education program
that you may be interested in. This means that a licensed, nurse case manager will be directly
involved in your health education. We believe this new approach will be more beneficial because
you can work directly with a case manager who will streamline and choose educational materials
to best meet your individualized goals.
Changing to an individualized educational experience will allow us to be more comprehensive
in a single mailing, give you more focused educational materials, and allow you more time to
engage with a nurse if you choose. This is all part of your employer's benefit plan and is no
cost to you.
Please note that you must speak to a case manager to be enrolled in the new program, so if you

providers, services or products. Check your member benefits for coverage of services. All information
furnished by you is kept strictly confidential and only used to provide us with information necessary
for participation in the Case Manger lead Health Education Program.
